About the Electronic Detailed Survey Form (eSS) The traditional paper-based detailed survey form has been converted into an electronic version (PDF), with input fields (text, checkboxes, radio buttons, calendars, etc.) set within the PDF. This implementation enhances the convenience of creating survey forms and includes logical checks during data entry. Creation of the form only requires the free Acrobat Reader; no other applications are necessary. During data entry, the check function can be used to detect omissions, inconsistencies, and violations in the input. After entry, the lock function prevents unauthorized modifications or tampering of the data. The standard version of the detailed survey form uses the format provided by the Pharmaceutical Manufacturers Association. Custom formats for each product can also be accommodated.

Sample Screen (1) Selection Input (List Box Selection): This is convenient for inputting a single choice from options (standard terms). You can select from the list box and input the desired term. Benefits: It ensures consistency in terminology.

The detailed survey form for the standard version uses the format provided by the Pharmaceutical Manufacturers Association, but we can accommodate product-specific formats (unique to pharmaceuticals) and custom formats from clients. Additionally, we can handle not only detailed surveys but also adverse event reporting forms, drug usage surveys, and other standard formats.
