**Lecture Overview** With the increase in the elderly population, the demand for home healthcare and the number of emergency transports are on the rise. On the other hand, when a patient's condition deteriorates at home, there are instances where the patient's wishes are not shared with family members or medical institutions, leading to the provision of unwanted medical care. Therefore, it has become a challenge to establish a collaborative system for sharing information about the patient's condition and desired medical care (such as preferred place of care and wishes regarding life-sustaining treatment) among relevant organizations in the community. Additionally, to respond to the increasing demand and maintain a quality healthcare provision system, strengthening collaboration is essential.
